Objectives: The objective of this study is to investigate the relationship between a physician’s subjective mortality prediction and the level of confidence with which that mortality prediction is made.Design and participants: The study is a prospective cohort of patients less than 18 years of age admitted to a tertiary Paediatric Intensive Care Unit at a University Children’s Hospital with a minimum length of ICU stay of 10 h. Paediatric ICU attending physicians and fellows provided mortality risk predictions and the level (...) of confidence associated with these predictions on consecutive patients at the time of multidisciplinary rounds within 24 hours of admission to the paediatric ICU. Median confidence levels were compared across different ranges of mortality risk predictions.Results: Data were collected on 642 of 713 eligible patients . Mortality predictions greater than 5% and less than 95% were made with significantly less confidence than those predictions 95%. Experience was associated with greater confidence in prognostication.Conclusions: We conclude that a physician’s subjective mortality prediction may be dependent on the level of confidence in the prognosis; that is, a physician less confident in his or her prognosis is more likely to state an intermediate survival prediction. Measuring the level of confidence associated with mortality risk predictions may therefore be important because different levels of confidence may translate into differences in a physician’s therapeutic plans and their assessment of the patient’s future. The paper presents fragments of a Byzantine icon discovered in 2015 during regular archaeological excavations carried out in Chełm, eastern Poland. Iconographic analyses allow the nine surviving fragments to be interpreted as belonging to a diptych wing with the Great Feasts cycle. The icon represents archaic iconography of the subject, with the scene of Transfiguration placed after Entry into Jerusalem and before the Crucifixion. The artefact was created in the second half or at the close of the 12th century, and (...) it was made from steatite, which has been confirmed by petrographic analyses. The icon was discovered in the remains of a palace complex of King Daniel Romanovich, the greatest ruler of the Galicia-Volhynia Lands. The results of the archaeological research allow the terminus ante quem for the icon’s arrival in Chełm to be determined as before the middle of the 13th century. Various possible explanations as to how the icon found its way to Chełm are also explored in the paper. (shrink)

Rationale, aims and objectives Given the increasing prevalence of cataract and demand for cataract extraction surgery, patients must often wait to undergo this procedure. We validated a previously developed priority scoring system in terms of clinical variables, pre-intervention health status, appropriateness of surgery and gain in visual acuity (VA) and health-related quality of life (HRQoL).Methods Explicit prioritization criteria for cataract extraction created by a variation of the Research and Development (RAND) and University of California Los Angeles appropriateness methodology were retrospectively (...) applied to a prospective cohort of 5257 patients on waiting lists to undergo cataract by phacoemulsification at 17 hospitals in Spain. Demographic data, clinical data and data related to surgical technique were collected by trained ophthalmologists. Patients were evaluated by their ophthalmologist before the intervention and 6 weeks afterward. They also completed, by mail, the Visual Function Index (VF-14) before the intervention and 3 months afterward.Results High-priority patients experienced greater improvement in VA and HRQoL than those classified as intermediate or low-priority (P < 0.0001), even after adjusting by VA and the VF-14 score at baseline. The time to intervention was the same for high-priority patients as it was for intermediate and low-priority patients.Conclusions The priority score we developed identified patients most likely to experience the greatest improvements from cataract extraction. Use of this tool could provide a fairer and more rational way to prioritize patients for cataract extraction. (shrink)

The coronavirus disease pandemic fundamentally disrupted humans’ social life and behavior. Public health measures may have inadvertently impacted how people care for each other. This study investigated prosocial behavior, its association well-being, and predictors of prosocial behavior during the first COVID-19 pandemic lockdown and sought to understand whether region-specific differences exist. Participants from eight regions clustering multiple countries around the world responded to a cross-sectional online-survey investigating the psychological consequences of the first upsurge of lockdowns in spring 2020. Prosocial behavior (...) was reported to occur frequently. Multiple regression analyses showed that prosocial behavior was associated with better well-being consistently across regions. With regard to predictors of prosocial behavior, high levels of perceived social support were most strongly associated with prosocial behavior, followed by high levels of perceived stress, positive affect and psychological flexibility. Sociodemographic and psychosocial predictors of prosocial behavior were similar across regions. (shrink)

Objective: Illness perceptions are important predictors of emotional and behavioral responses in many diseases. The current study aims to investigate the COVID-19-related IP throughout Europe. The specific goals are to understand the temporal development, identify predictors and examine the impacts of IP on perceived stress and preventive behaviors.Methods: This was a time-series-cross-section study of 7,032 participants from 16 European countries using multilevel modeling from April to June 2020. IP were measured with the Brief Illness Perception Questionnaire. Temporal patterns were observed (...) considering the date of participation and the date recoded to account the epidemiological evolution of each country. The outcomes considered were perceived stress and COVID-19 preventive behaviors.Results: There were significant trends, over time, for several IP, suggesting a small decrease in negativity in the perception of COVID-19 in the community. Age, gender, and education level related to some, but not all, IP. Considering the self-regulation model, perceptions consistently predicted general stress and were less consistently related to preventive behaviors. Country showed no effect in the predictive model, suggesting that national differences may have little relevance for IP, in this context.Conclusion: The present study provides a comprehensive picture of COVID-19 IP in Europe in an early stage of the pandemic. The results shed light on the process of IP formation with implications for health-related outcomes and their evolution. (shrink)

Conducting research during or in the aftermath of disasters poses many specific practical and ethical challenges. This is particularly the case with research involving human subjects. The extraordinary circumstances of research conducted in disaster settings require appropriate regulations to ensure the protection of human participants. The goal of this study is to systematically and qualitatively review the existing ethical guidelines for disaster research by using the constant comparative method. We performed a systematic qualitative review of disaster research ethics guidelines to (...) collect and compare existing regulations. Guidelines were identified by a three-tiered search strategy: 1) searching databases, 2) an Internet search, and 3) a search of the references in the included documents from the first two searches. We used the constant comparative method for analysis of included guidelines. Fourteen full text guidelines were included for analysis. The included guidelines covered the period 2000-2014. Qualitative analysis of the included guidelines revealed two core themes: vulnerability and research ethics committee review. Within each of the two core themes, various categories and subcategories were identified. Some concepts and terms identified in analyzed guidelines are used in an inconsistent manner and applied in different contexts. Conceptual clarity is needed in this area as well as empirical evidence to support the statements and requirements included in analyzed guidelines. (shrink)

It has been widely recognized that music and poetry sprang from the common origin of chant or incantation.1 Throughout the ages, the sister arts sometimes went hand in hand and sometimes parted company, but since the end of the nineteenth (...) century musical aspects have been used quite extensively in literature, either as a subject matter or as a wellhead of structures. The number of musically inspired twentieth-century novels bears evidence to this strengthening of musico-literary relationships.2 Contemporary interactive, interdisciplinary, and multimedial works of art or artistic events also testify to a close sisterhood between the arts. This phenomenon has come to be called intermediality, which is defined as using more than one artistic medium in the creation of a work of art. But then, examining musical aspects in literature demands specific knowledge of the musical field itself, thus raising questions about the limits and the difficulties of using musico-literary materials in class. Suppose the student is not knowledgeable at all about music. What, then, can be the input the teacher can offer the student without necessarily inundating the latter with too much information? Is it possible to avoid generalizations in drawing comparisons or distinctions between music and literature? And, is it relevant to deal with music in teaching fiction?I wish to suggest that there are pragmatic elements of music that may be used in teaching music-related texts without previous skills in the musical field. In this respect, music may serve as an enriching and fascinating teaching tool. The point I would like to make is that quite often music leads the text to raise questions about literariness itself, so that it becomes possible not only to discuss musico-literary interrelations but to tackle the specificity of literature through the prism of intermediality. On the other hand, I wish to broach the question of the musicalization of fiction, that is to say, the transposition or imitation of musical forms in literature.Literature Speaking about MusicThe Need of a ContextIn dealing with musical contexts of literary texts, the teacher may bring the student's attention to bear on the variety of ways a text may be related to music. A typology of different musico-literary interrelations has been established by such critics as Calvin S. Brown3 or Steven P. Scher. In Scher one will find a useful chart dividing musico-literary studies into two categories: literary presence within the musical field (explored by musicology) and musical presence within the literary field (explored by literary studies). The latter may be subdivided into word music, musical structures, and verbal music. This typology has been furthered by Werner Wolf in his critical work on the musicalization of fiction. Yet too detailed a categorization may seem difficult to put into practice in a classroom setting. Its principle drawback lies in the difficulty to sever one musical phenomenon in literature from the other. In numerous cases it seems impossible to drive an unequivocal wedge between (a) prosodic phenomena, appertaining to word music or musicality; (b) exclusively thematic or topical elements; and (c) specifically structural transpositions. Such an accurate classification, therefore, preferably would be used with more advanced students.The thematic or topical relationship I want to focus on in this section is by far the easiest to get across to students since it does not require musical skills. This approach is roughly tantamount to Scher's verbal music or Wolf's thematization,4 and it appears in texts dealing with music explicitly. We study the forcing notions of Borel sets modulo the respective σ-ideals I and I* and find connections between their forcing properties. To this end, we associate to a σ-ideal on a Polish space an ideal on a countable set and show how forcing properties of the forcing depend on combinatorial properties of the ideal. We also study the 1—1 or (...) constant property of σ-ideals, i.e., the property that every Borel function defined on a Borel positive set can be restricted to a positive Borel set on which it either 1—1 or constant. We prove the following dichotomy: if I is a σ-ideal generated by closed sets, then either the forcing P I adds a Cohen real, or else I has the 1—1 or constant property. (shrink)

Based on the new experimental evidence, we argue that a link between a conditional antecedent and the consequent is semantically expressed rather than pragmatically conveyed. In our paper, we focus on particular kinds of links which conditionals may convey in a context. For instance, a conditional ‘If p, q’ may convey a thought equivalent to ‘p will cause q’, ‘p is the best explanation for q’, ‘q follows from p’, etcetera. The traditional theoretical literature on conditionals seems to imply that (...) these specific links are generated pragmatically and are akin to conversational implicatures. In order to test this hypothesis, we used a well-recognized linguistic test from ‘reinforceability’, which serves to distinguish between a semantic and pragmatic level of meaning, and we designed an experimental study based on that test. The outcome of our study is that specific links conveyed by conditionals exhibit features of semantic entailments rather than conversational implicatures. This result accords with some of the recent findings in empirical investigations on conditionals. In the final part of our paper, we discuss various accounts of conditionals which can accommodate the results of our study. (shrink)

In Darwin’s Dangerous Idea, Daniel Dennett claims that evolution is algorithmic. On Dennett’s analysis, evolutionary processes are trivially algorithmic because he assumes that all natural processes are algorithmic. I will argue that there are more robust ways to understand algorithmic processes that make the claim that evolution is algorithmic empirical and not conceptual. While laws of nature can be seen as compression algorithms of information about the world, it does not follow logically that they are implemented as algorithms by physical (...) processes. For that to be true, the processes have to be part of computational systems. The basic difference between mere simulation and real computing is having proper causal structure. I will show what kind of requirements this poses for natural evolutionary processes if they are to be computational. (shrink)

W kontekście kryzysu wywołanego rozwojem nowoczesności stawiam pytanie o możliwość podjęcia działań prewencyjnych. Utrzymuję, że ich skuteczność zależy od tego, czy możliwe jest spojrzenie na obecną sytuację z perspektywy zewnętrzności (transgresja), do której odnoszą się światopoglądy często pozostające ze sobą w konflikcie. Twierdzę, że świeckie państwo stwarza szansę na owocny dialog tych światopoglądów stwarzając szansę na realizację działań prewencyjnych.

Psychonarratology is an approach to the empirical study of literary response and the processing of narrative. It draws on the empirical methodology of cognitive psychology and discourse processing as well as the theoretical insights and conceptual analysis of literary studies, particularly narratology. The present work provides a conceptual and empirical basis for this interdisciplinary approach that is accessible to researchers from either disciplinary background. An integrative review is presented of the classic problems in narratology: the status of the narrator, events (...) and plot, characters and characterization, speech and thought, and focalization. For each area, Bortolussi and Dixon critique the state of the art in narratology and literary studies, discuss relevant work in cognitive psychology, and provide a new analytical framework based on the insight that readers treat the narrator as a conversational participant. Empirical evidence is presented on each problem, much of it previously unpublished. (shrink)

What is considered normal determines clinical practice in medicine and has implications at an individual level, doctor-patient relationship and health care policies. With the increase in medical information and technical abilities it is urgent to have a clear concept of normality in medicine so that crucial discussions can be held with unequivocal terms.The different meanings for normality were analyzed throughout the literature and grouped according to their relevance in the academic community in models, namely the Biostatistical Theory (BST), Health, Ideal, (...) Process and Biological advantage. The BST is the most established naturalistic approach, however normal variability can arguably constitute a problem. Health is similar and raises the question of setting the boundaries of pathology. Normality as an Ideal is an useful tool but is naturally unrealistic. As a Process it is comprehensible but is hard to frame for practical purposes. If considered as a Biological Advantage, seems intuitive but abnormality should tend to disappear.After, three examples were presented to discuss these models. They were Anemia, Psychiatric diseases and Psychopathy. In the case of Anemia the BST was applied and the arbitrary boundaries but with social impact were exposed. Psychiatric diseases was discussed under the process of self-organization and non-suffering ideal. With Psychopathy the boundaries of biological advantage are questioned.This review appeals to the importance of redesigning of the concept of normality in medicine according to current times and stresses the importance of integrating concepts such as variability and autonomy. This paper critically investigates the implementation of the UN guiding principles on business and human rights (UNGPs) into the corporate setting through the concept of ‘translation’. In the decade since the creation of the UNGPs, little academic research has focussed specifically on the corporate implementation of human rights. Drawing on qualitative case studies of two multinational corporations—an oil and gas company and a bank—this paper unpacks how human rights are translated into the corporate context. In doing so, the paper focuses (...) on the “resonance dilemma” translators encounter, the strategies used to make human rights understandable and palatable, and the difficulties that emerge from this process. We contend that the process of making human rights understandable and manageable can change their form and content, which may act as an obstacle to human rights realisation and corporate accountability for human rights. This paper offers a new way to make sense of disagreement expansion from a polylogical perspective by incorporating various places in addition to players and positions into the analysis. The concepts build on prior implicit ideas about disagreement space by suggesting how to more fully account for argumentative context, and its construction, in large-scale complex controversies. As a basis for our polylogical analysis, we use a New York Times news story reporting on an oil train explosion—a significant point in the (...) broader controversy over producing oil and gas via hydraulic fracturing. In this study, we examine the accounts of 30 white middle- and upper-class female heroin/methadone users. Using a resistance framework, we note that these women recall their initial heroin use in ways that suggest rejection of restrictive gender and class expectations. Using a dynamic view of resistance, we begin to understand how these women attempt to resist the dominant discourse through their heroin use and to reinterpret their experiences with heroin.

W 1983 roku Benjamin Libet wraz ze współpracownikami po raz pierwszy wykazał, że w prostym działaniu dobrowolnym świadoma intencja nie pełni funkcji inicjującej. Czasowy przebieg tego typu działania wskazuje również, że intencja oraz samo działanie to produkty procesów nieświadomych. Na podstawie wyniku Libeta oraz wybranych koncepcji psychologicznych Daniel Wegner zaproponował teorię pozornej mentalnej przyczynowości, w ramach której intencja to rodzaj konstruktu umożliwiającego agentowi zrozumienie własnego zachowania w kategoriach przyczynowych, gdzie jego stan mentalny (intencja) jawi mu się jako przyczyna, a działanie (...) jako skutek. Ujęcie Wegnera rodzi jednak pytanie, jaka jest faktyczna relacja pomiędzy intencjami, a celami kształtującymi nasze decyzje. Biorąc pod uwagę wyjaśnienie amerykańskiego psychologa, nie możemy być pewni, że te dwa elementy mają ze sobą jakiś związek. Może to prowadzić do zaskakującego wniosku, że nasza świadoma wola jest w gruncie rzeczy iluzją. Korzystając z wyników badań Read’a Montague nad mechanizmami ustanawiania i osiągania celów wskazuję na alternatywną możliwość rozumienia roli intencji. Zgodnie z analizą Montague, możemy postulować, że przynajmniej w wybranych przypadkach treść intencji działania oraz przebieg zachowań celowych oparte są na tej samej abstrakcyjnej reprezentacji. Znaczy to, że interpretacja świadomej woli, jako przydatnej iluzji jest za szeroka, gdyż znamy (wskazane w tekście) przypadki, kiedy treść intencji jest tożsama z reprezentacją pełniącą rolę nagrody w systemie ustanawiania i osiągania celów. Virtue could be used to impart moral authority to arguments about political power. The development of this strategic idea is traced through the works of key Enlightenment thinkers. There is also consideration of the ways in which numerous popular writers of the day, including clerics, eulogists, journalists, novelists and lawyers, employed the idea of virtue in polemical discussions in their (...) writings. Explanations in cognitive science and computational neuroscience rely predominantly on computational modeling. Although the scientific practice is systematic, and there is little doubt about the empirical value of numerous models, the methodological account of computational explanation is not up-to-date. The current chapter offers a systematic account of computational explanation in cognitive science and computational neuroscience within a mechanistic framework. The account is illustrated with a short case study of modeling of the mirror neuron system in terms of predictive coding.

In his recent book, Daniel Dennett defends a novel account of semantic information in terms of design worth getting (Dennett, 2017). While this is an interesting proposal in itself, my purpose in this commentary is to challenge several of Dennett’s claims. First, he argues that semantic information can be transferred without encoding and storing it. Second, this lack of encoding is what makes semantic information unmeasurable. However, the argument for both these claims, presented by Dennett as an intuition pump, is (...) invalid. (shrink)

The discussion around dignity in nursing philosophy has been underway for many years. The literature still lacks philosophical arguments that would justify the thesis that all people have dignity. Scholars who defend dignity as an intrinsic value most often refer to Kant. However, Kant does not seem to be the most suitable candidate to defend the thesis that all human beings possess dignity. In this paper, I attempt to show that Aristotle's and Aquinas's views can help justify this thesis. Following a discourse analysis approach this study examines triadic encounters of nine medical consultations carried out in an outpatient cancer clinic in Santiago, Chile involving an oncologist, a patient and a patient’s companion. Consultations are fully transcribed and analysed to understand the participatory roles patients’ companions play in the medical exchanges. In addition, the type of linguistic functions associated with each role and the advantages or downsides of companions’ involvement in the encounters are also investigated. The analysis reveals seven prominent (...) roles that companions may engage in during the visits. These are studied in detail. The results show that although companions may not always provide information that is consistent with the patients’ views, they can be of great help. Relieving the patients of many daunting tasks, monitoring their well-being, establishing a partnership and bringing vital information to the clinic are instances where the controlling power that some may exercise in the medical encounter can be outweighed. Lipski's book on KOR can be read from one main perspective: how does opposition come about? This does not refer to opposition in general, but to opposition within totalitarian or nearly totalitarian conditions. Such conditions are interesting because they rule out social action by autonomous self-motivated subjects. Lipski answers the question directly in the chapter on “The Prehistory of KOR.” Birth is preceeded by a long process of formation of intellectuals' circles and the strengthening of social, occupational, and experiencial links, (...) e.g., the role of post-AK circles. En ese artículo de naturaleza bibliográfica explicamos porque el diálogo intercientífico permite que se transcienda la homogeneidad de una epistemología involucrada en sí misma, expandiéndose para la transdisciplinariedad cultural y epistemológica. Analizamos dos nuevos modelos de paradigma alternativo, a saber: el vivir bien y el desarrollo endógeno sustentable. Consideramos que esas concepciones filosóficas tienen el desafío de desarrollar nuevas posibilidades epistemológicas y metodológicas que permitan el cambio de la realidad social e histórica. The starting-point of my essay is conviction of wide-spread crisis of western civilization. The analysis of this crisis was achieved basing on the Horkheimer's theoretical settlements which are leading to show typical of Plato character of western culture. Basing on the example of 'solar myth' presented by Plato in "Politeia", it is shown totalitarian and captivating human-beings paideia model, which is being realized till the present day in the Mediterranean culture region. Dictatorial, anti-humanistic and oppressing elements of Platonism and related (...) to it civilization, are shown from the G. Bataille's settlements point of view. In the newly established state marine traditions of Byzantines, remain alive. The best testimony to this, are the evidence contained in the chronicle of Georgios Akropolites, devoted to activities of the rulers of Nicaea, aimed to build their own naval forces. In this paper I'll also try to answer, where was beating the heart of the Nicean shipbuilding (...) industry and how large was the navy of this state. This is important from point of view of the maritime history, because of the fleet of the Empire of Nicaea, filled the gap created after the fall of Constantinople and the Byzantine Empire, which was the local naval power in previous centuries. Akropolites give us a clear and direct answer to a question, where we should search for a center of Nicaean shipbuilding industry. Georgios Akropolites suggest us, that was in two towns, Holkos and Smyrna. The above-mentioned fleet consisted of the few squadrons, each counting 5-6 ships. We can only guess that a fleet of the John III, could count about 50 warships, whose quality was worse to that belonging to the Venetians. We must say that the fleet of the Empire of Nicaea, which we see in the chronicle of Akropolites, was the force, that lent itself to the support of ground forces. And in this role worked well. The situation was different when it comes to clashing with the Venetians, with the experienced crews of their ships, who surpassed Nicaean in this matter. Even with the advantage of numbers, Nicaean was unable to overcome at the sea, the citizens of the Republic of St. Mark. The plan to build their own naval forces, which was taken by the emperors of Nicaea, was a good direction. (shrink)
